As a medical model adult day program, Day-Break has the resources and structure necessary to accommodate the needs of many seniors—from assisting with ADLs to insulin administration and Alzheimer’s support. At Day-Break we help our clients follow a general healthy lifestyle, keep to their medication regimen and comply with their doctors' recommendations. This is especially important for patients needing extra assistance for chronic conditions, or recuperating from hospitalization.
The structure and support of an adult day center can often help reduce the chances of readmission to the hospital or nursing home. At Day-Break we monitor key health metrics, including changes in appetite and energy, and can report and respond to changes before they escalate. Our social activities, assistance with hygiene and ADL’s enhance our client’s willingness to follow a healthy regimen designed for recovery or improved day-to-day health management.
The Day-Break staff also assits families and professionals with coordination and referrals to other services such as housing, home delivered meals, transportation, and more. With us, you can be sure that the people you refer are being cared for safely and reliably, and at a lower cost than most in-home providers.

Each brings to his/her position a diverse set of skills and insights, but the team works together to achieve one cohesive goal: to help our members live healthier, happier lives.
Our director brings experience and warmth of personality every day to the job, and enjoys working with the Day-Break seniors and their families to develop client-centered plans that recognize the unique knowledge, talent and strength of each individual.
Our nurses relish the interaction between family, caregivers, clients and care managers, which gives them the opportunity to work with Day-Break clients and families to better under stand health issues such as diabetes, nutritional counseling and hypertension.
